This beloved creation is one of the most ubiquitous American menu items and usually comes with fries. The oldest recorded recipe for it was published in Good Housekeeping Everyday Cook Book in 1903. Traditionally, it includes toasted sandwich bread stacked with sliced cooked chicken or ham, bacon, lettuce, tomato and mayonnaise.
